Transfer Request Form

All transfer credit requests must be completed within the student's first year of admission into the degree program. No transfer request forms will be accepted for review for non-degree seeking students.

Pre-Approval Request Form 

This form is used to pre-approve coursework at another university that will be used to satisfy graduation requirements.  A number of restrictions apply; please review the instructions on the form.

Graduate Change of Program Request

This form is used to change a major within the School of Management. This form must be completed by degree-seeking students wishing to change their majors. Please note the change of major form can not be used to add a second degree; a separate admissions application must be submitted.
