Application Instructions,
Step-by-Step

A complete application must be sent to the Executive MBA program office by regular mail or email, using the addresses listed below. You may send forms together in a single packet or separately. Before you complete any forms you should read the UTD policy statements related to submitting information to UT Dallas.

Step 1: Submit completed application along with $1300 application fee/deposit. Click on these forms to display and complete them:

  • Application Form (Don't forget to send us the typed, Personal Statement requested on page 3.)

  • Sponsorship Form

  • Recommendation Forms (Three are required. Your references must be sent directly to the Executive MBA office.)

  • Contact Connie Imhof at 972-883-5983 to pay by credit card or send a check made payable to "UT Dallas" and send to address below.

These are Microsoft Word forms. You should complete them online. Then you may: (1) save them to your desktop, then attach them to an email and send this email to the address below; or (2) print them out, make copies for yourself, sign them, and mail to the address below.

Step 2: Order official transcripts. An official, sealed transcript is required from all universities/colleges you have attended. Transcripts must be sent directly to us. This Transcript Request Form can be used.

Some universities allow transcripts to be ordered directly from their website. Instruct the institution to mail transcript to the address above.

Step 3: Arrange an interview. After you have completed your application, contact Connie Imhof at cimhof@utdallas.edu or 972-883-5983 to schedule an interview. Bring a current resume to the interview.
